What Is Ectopic Pregnancy?

In a normal pregnancy the fertilised egg travels down the fallopian tube and implants in the lining of the uterus. In an ectopic pregnancy it implants somewhere else — most commonly in the tube itself, and occasionally in the ovary, cervix, a caesarean scar or the abdomen.

None of these locations can support a growing pregnancy. The tube in particular cannot stretch, and as the pregnancy grows it may rupture, causing bleeding into the abdomen.

An ectopic pregnancy cannot be moved into the uterus. Treatment is about ending it safely and protecting your health and future fertility.

Who Is More at Risk?

Many women who have an ectopic pregnancy have no risk factors at all. Where they exist, the common ones include:

  • Previous tubal damage or blockage
  • Previous pelvic infection or pelvic inflammatory disease
  • Endometriosis or previous pelvic surgery
  • A previous ectopic pregnancy
  • Conception with an intrauterine device in place
  • Smoking

Having a risk factor does not mean it will happen. Not having one does not rule it out.

How Is Ectopic Pregnancy Diagnosed?

Diagnosis is usually straightforward once you are seen, and often settled within a day or two:

  • Transvaginal ultrasound: looks for a pregnancy inside the uterus and for signs of one outside it, or free fluid in the pelvis.
  • Serial beta hCG blood tests: in a healthy early pregnancy this hormone rises in a predictable pattern; in an ectopic it often rises slowly or plateaus.
  • Clinical assessment: pulse, blood pressure and abdominal examination, to judge whether this is stable or urgent.
  • Laparoscopy: where the picture remains unclear, or where you are unstable and treatment cannot wait.

Early on, a scan may not be able to locate the pregnancy at all — this is called a pregnancy of unknown location, and it is followed closely with repeat tests rather than ignored.

Ectopic Pregnancy Treatment Options

Which treatment is right depends on how far the pregnancy has advanced, your hCG level, whether the tube has ruptured and how stable you are.

  • Expectant management: in carefully selected early cases with falling hCG, close monitoring may be enough.
  • Medical treatment with methotrexate: an injection that stops the pregnancy tissue growing, avoiding surgery. Suitable only for stable women within certain limits, and requires follow-up blood tests.
  • Laparoscopic surgery: keyhole surgery to remove the pregnancy, preserving the tube where that is possible (salpingostomy) or removing the tube where it is too damaged (salpingectomy).
  • Emergency surgery: where the tube has ruptured and there is internal bleeding, surgery happens without delay.

Laparoscopic Surgery for Ectopic Pregnancy

Where surgery is needed and you are stable, it is usually done laparoscopically — through small incisions, with a shorter recovery than open surgery and less scarring inside the pelvis.

Whether the tube can be saved depends on how damaged it is and on the state of the other tube. Preserving a badly damaged tube is not always in your interest: it carries a higher risk of another ectopic pregnancy. That trade-off is explained to you rather than decided silently.
