What Is Male Infertility?
Male infertility means a problem with the production, quality or delivery of sperm that makes conception difficult.
It is common, it is frequently treatable, and it is almost never something a man can tell from how he feels. There are usually no symptoms at all — which is exactly why testing rather than assuming is the only sensible approach.
A male factor contributes to roughly half of all infertility, sometimes as the only cause and often alongside a female factor.
What Causes Male Infertility?
Causes fall into a few broad groups:
- Varicocele — enlarged veins in the scrotum, one of the more common and more treatable causes
- Hormonal problems — affecting the signals that drive sperm production
- Obstruction — a blockage anywhere along the path sperm travel, including after infection or previous surgery
- Infection — past or present, affecting the testes or the ducts
- Genetic factors — including chromosomal causes affecting sperm production
- Lifestyle and environment — smoking, alcohol, obesity, heat exposure, certain medications and anabolic steroids
In a proportion of men no cause is identified. That does not mean nothing can be done — treatment is guided by the semen analysis rather than by the label.

The absence of symptoms tells you nothing about fertility. A semen analysis is the only way to know, and it is a straightforward test.
How Is Male Infertility Diagnosed?
Evaluation is usually quick and starts with the simplest test:
- Semen analysis: measures the number of sperm, how well they move and how they are formed. Because results vary naturally, an abnormal report is normally repeated before conclusions are drawn.
- Medical and lifestyle history: previous surgery, infections, medications, occupation and habits.
- Physical examination: including assessment for a varicocele.
- Hormone blood tests: where sperm counts are low, to check the hormonal signals driving production.
- Scrotal ultrasound: to confirm a varicocele or look for obstruction.
- Genetic testing: considered where counts are very low or absent.
We run semen analysis in our in-house andrology laboratory, so the sample is handled on site rather than transported — which matters, because handling and timing affect the result.
Male Infertility Treatment Options
Treatment follows the cause, and ranges from simple measures to assisted reproduction.
- Lifestyle change: stopping smoking, reducing alcohol, weight loss and avoiding heat exposure genuinely improve parameters for many men.
- Medical treatment: for hormonal causes or infection.
- Surgery: varicocele repair, or correction of an obstruction.
- IUI: where parameters are mildly reduced, washed and prepared sperm can be placed directly into the uterus.
- IVF with ICSI: a single sperm injected directly into the egg — the mainstay where counts or motility are significantly reduced.
- Surgical sperm retrieval: where no sperm are present in the ejaculate, sperm can sometimes be recovered directly for use with ICSI.
Choosing Between IUI, IVF and ICSI
The semen analysis largely decides this. Mildly reduced parameters often respond to IUI, particularly where the female evaluation is normal and the woman is younger. Significantly reduced count, motility or morphology point towards IVF with ICSI, because ICSI bypasses the need for sperm to reach and penetrate the egg on their own.
There is no benefit in working through the options in order for the sake of it. Where IUI has a poor chance from the start, we will say so rather than let you spend months and money finding out.
What the Decision Depends On

- Sperm count, motility and morphology on a repeated analysis
- Whether a treatable cause such as a varicocele or infection is present
- Your partner's age and ovarian reserve
- Whether her tubes are open
- How long you have been trying
- Previous treatment and its outcome
- Your own preferences on cost, time and invasiveness
